What is a disability?
Disabilities may include hearing, mobility or visual impairments as well as hidden disabilities such as chronic medical conditions, learning, psychiatric or emotional disabilities.
Request for Accommodations
- The student must self identify as having a disability
- Complete an application for accommodations (PDF)
- Provide documentation of the disability by a licensed professional
- Provide a current class schedule
- Sign student responsibility form (PDF)
- Take accommodation notification form (s) to instructors
